- 4 Chronic pain related to a musculoskeletal disorder or of ‘visceral’ origin
- 5 ALS
- 6 Terminal cancer
- 7 Opioid use disorder
- 8 Muscular dystrophy
- 9 Tourette’s syndrome
- 10 Terminal illness
- 11 An inflammatory bowel disease such as Crohn’s disease
- 12 Glaucoma
- 13 PTSD
- 14 Intractable skeletal, muscular spasticity
- 15 A seizure disorder including epilepsy
- 16 Cancer
- 17 HIV/AIDS
